REPAIR / MAINTENANCE REQUEST  |  MR1

A great “before” and “after” format form to better manage repair requests in your fleet or repair shop. Our Repair / Maintenance Request | MR1 form allows operators to document their unit’s issues, then turn over the form to the shop for reference once work begins. Your shop mechanic can record work done, as well as parts / consumables used, on the bottom half of the form before closing the repair job.

Maintenance Request forms are used to help operators provide clear and precise instructions to maintenance staff. This is a great step to have prior to your maintenance work orders to ensure that your vehicles and equipment are always in working order. Most common information included on maintenance requests include vehicle, equipment and driver details and description of issues needing attention. Some request forms may continue on to include details on the work performed in order to see work through to completion.
