DRIVER’S DAILY LOG  |  DDL5

DDL5 provides both a Driver’s Daily Log (for both 70-hour and 120-hour cycles) and a Truck & Trailer Daily Inspection. The lower half inspection report follows Schedule 1 guidelines and provides ample space for writing in remarks. You can feel comfortable knowing that you’ve provided all the required information on your Driver’s Daily Log (e.g. driver name; addresses for home terminal and principal place of business; unit and license plate numbers for both truck and trailer; total hours for each duty status; total hours in the day; cumulative cycle; start time of the day; driver signature; etc.) as laid out in Commercial Vehicle Drivers Hours of Service Regulations.
Note: users should retain a complete copy of Schedule 1 in their units for reference.

Standard customization options include:

• Company logo
• Company header (address, phone, etc.)
• Principal place of business address (log section)
• Carrier name (inspection section)
• Form numbering (two locations)
