JSA & Meeting Report | JSA2 is a large, legal-sized (8.5″ x 14″) form that starts with properly documenting your worksite details such as company, contractor, location and site supervisor. After that, identify your job assignment specifics. Then formulate your task / hazard / control approach with the spacious table provided. With room for up to 24 attendee names, this template is ideal for safety meetings at locations where multiple crews are active.

Hazard assessments are an essential part of every corporate health & safety program. Not only is it a requirement of Occupational Health & Safety regulations to perform hazard assessments but they must also be properly documented as well. A Job Safety Analysis, also known as a Task Hazard Assessment, are completed in order to identify potential or existing hazards that may be present during the course of a specific job or task. The assessor will then put in place and record preventative measures to eliminate or mitigate the hazards.
