This letter-size Ground Disturbance Checklist | GDC2 template is designed to facilitate safe and compliant ground disturbance activities. It begins by collecting essential project details such as date, job location, job description, customer/company name, and the number of workers. The core of the form is a comprehensive checklist, presented in a two-column table format. These checklist items cover critical safety aspects, including the completion of hazard assessments, worker training, contact with consultants, supervisors, and landowners, communication with provincial utility locate services and other companies, review of survey plans, and the presence of an Emergency Response Plan and Muster Point. It also addresses pre-job meetings, SDS sheet availability, PPE, excavation dimensions, identification of existing markers, overhead power lines, the presence of a Ground Disturbance Supervisor, availability of barricades, hand exposure of lines, excavation depth, soil conditions, topsoil/subsoil management, environmental considerations, bell hole access, spoil pile distances, and firefighting equipment. Additionally, it prompts for confirmation that visible signs of underground facilities have been located.
