Field Hazard Assessment | HA1 features a handy check box format to document hazards. Listing includes 45 of the most common hazards as well as 5 blank “other” fields. Next is 8 lines for documenting your job tasks, associated hazards and control plans. The personnel table can hold the names and signatures of up to 10 crew members and there is still ample space (4 full width rows) at the bottom of the page for your job foreman to write out additional comments or notes.
Hazard assessments are an essential part of every corporate health & safety program. Not only is it a requirement of Occupational Health & Safety regulations to perform hazard assessments but they must also be properly documented as well. A Field Level Hazard Assessment, also known as an FLHA, identify potential or existing hazards that may be present on and around your worksite or location. The most senior person completes it before the start of any work and are to update or reassess if any changes to personnel, equipment, tasks, etc.
