INCIDENT REPORT | IR2
The Incident Report | IR2 form is a letter-sized form designed for comprehensive documentation of various incidents. It allows for recording the date and time of the incident, its location, and the type of incident from a checklist which includes a wide range of options to choose from. The form provides ample space for Incident Details, where users can specify parties involved (including workplace roles), work activities being performed, tools and equipment in use, and worksite conditions at the time of the incident. A detailed description of the incident can be provided, and there’s a section for a diagram of the worksite. The report also includes a table for Corrective Actions, with columns for Contributing Factors to Incident, Corrective Action Required, Assigned To, and Date Completed. Finally, a ‘FOR OFFICE USE ONLY’ section allows for noting if the incident has been reported to external authorities such as OH&S, CANUTEC, WCB, or Other.
