Our Field Level Hazard Assessment Card | FLHA5C template is built on a full 8.5″ x 11″ page size. It contains 11 rows for recording tasks, hazards and controls followed by our unique icon-based PPE checklist. This makes it easy to visually identify up to 10 of the most common pieces of worksite safety equipment. Then collect up to 10 worker sign-offs. The back side provides a listing of 70 common construction and industrial hazards to assist the author in a thorough hazard identification – broken down into environmental, ergonomic, equipment / tool, access / egress, rigging / hoisting, energy, personal limitation and safety administration categories.
