HAZARD ID / NEAR MISS CARD | HID1C
With our Hazard ID / Near Miss Card | HID1C you can easily confirm the type of event that occurred in order to determine how the information should be processed. Prior to the four full width lines to describe your recordable event are fields for Report Date, Location and Reported By. Following that is an easy-to-use checkbox listing of 18 common categories to help ID root causes of the event. You can go on to document the recommended actions, personnel responsible for completing those actions and a supervisor’s sign-off.
Standard customization options include:
- Company logo
- Company header (address, phone, etc.)
- Form numbering
